Guidance for Completing Your 学院年报

- Significant Financial Conflict of Interest

The required Conflicts Disclosures include the annual 外部活动/承诺冲突和重大财务利益冲突. We ask that you report on the most recent calendar year. 对于2022年1月到期的报告，请在2021年春季，夏季和秋季报告.

Interfolio can be accessed via your 集市门户. Once in Agora, look for the link to “院系年报" under "学术及课程.”

You will be taken to a screen that shows all of your prior 形式. You can view 形式 by clicking on the eye icon at the far right under 行动.
是的! You can actually access your activities year-round. The annual reporting period asks for updates from the prior calendar year, but we encourage you to update your activities throughout the year, you may update current or historical entries at any time.

是的! Look for the pencil icon next to your entries. 点击 the icon and you will be able to edit the entry.
If your course list is missing courses that you taught, please email bcfarhelp@dctdsj.com 附下列资料:

1. Look for the 预览 button on any of the form landing pages.
When you click on the preview button, you will get a pop up box:

2. You may also download a printable version of your report via Vitas和biosketch 在左侧导航栏. 一旦进入Vitas和生物草图页面，点击教师年度报告右侧的眼睛图标.

Navigate to your submitted 形式 through the 表格及报告 option in the left side menu and under the 发起活动输入表格 you will find all of your submitted 形式. 点击 the form that you need to edit.

You may also check the status of your form by going into the 表格及报告 点击屏幕 发起活动输入表格 which will show the status of your 形式.
